29 CFR 1910.219(d)(1):     Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    a.  Located on the skip hopper's chute, there were four 9 inch pulleys that were not guarded that were used to lift the chute in order to pour casting product into a blasting machine, thereby exposing the employees to ingoing nip points due to the unguarded pulleys.

29 CFR 1910.219(d)(3):     Broken pulleys. Pulleys with cracks, or pieces broken out of rims, shall not be used.    a. Located on the skip hopper's chute, there was a 9 inch pulley that had damage to the pulley's rim that was used in part to lift the chute in order to pour casting products into a blasting machine.  The damage to the pulley's rim exposed the employees to struck-by and/or caught-between injuries.

29 CFR 1910.219(f)(3):     Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.        a.  Located on the back of the skip hopper, there was a drive motor that operated a chain and sprocket that was not fully guarded that was used to rotate the drum of the skip hopper, thereby exposing the employees to a the ingoing nip point of the chain and sprocket.

29 CFR 1910.303(b)(1)(iv):     Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees.  Safety of equipment shall be determined using hte following considerations:      Electrical insulation:    a.  Located at the skip hopper, there was a  standard metal guardrail that had an electrical cord that wrapped around its top rail that was once used in part to supply electrical power to an interlock.  The electrical cord had damage to its outer jacket, thereby exposing any employee who made physical contact with the electrical cord or the standard guardrail.
