29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 11/26/2012, two employees were on an aerial lift without being attached to the basket. NOTE: RCC Associates, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, which was contained in OSHA inspection number 312147358, citation number 1, item number 1 and was affirmed as a final order on July 24, 2008, with respect to a workplace located at 1475 SW 5th St. Pembroke Pines, FL 33027.
