29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about November 6, 2012, two employees were providing wireline services during the completion of an oil/gas well. One of the employees was not wearing a hard hat. This exposed the employee to the hazard of being struck by falling objects.
