29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On or about november 20, 2012, at West Creek in League City, TX; the employer did not ensure that his employee was wearing the safety glases when he was using the circular saw to cut the wood for the cornice of the house being built. This condition exposed the employee to be struck by hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) On or about November 20, 2012, at West Creek in League City, TX; the employer did not ensure that his employee had fall protection when he was walking/working on a pumped jack scaffold. The worker was doing the cornice of the house. This condition exposed him to a 15 feet fall hazard onto the ground.
