29 CFR 1926.502(f)(2)(i): The rope, wire, or chain was not flagged at not more than 6-foot (1.8 m) intervals with high-visibility material: While work was being performed on the flat roof of a commercial building, and employees were protected from falls using the safety monitoring method of fall protection, the flags on the warning line along the north side of the roof were bunched together rather than being spaced at the required intervals.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface: Where employees performed roof work on a flat roof under the fall protection of a safety monitoring system, the warning line sagged in some areas to less than 34 inches.
