29 CFR 1926.403(b)(2):  Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a) On or about October 24, 2012; second floor; employees were using a power strip/tap to provide power to portable equipment either directly or through portable extension cords. The power strips/taps are not designed for outdoor or construction sites. The use of the equipment in this environment exposes the employees to an electrical shock.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    a) On or about October 24, 2012; rear of building; employees were exposed to a fall of approximately 30-40 feet when accessing the working platform by climbing the scaffold frame.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
