29 CFR 1926.102(a)(2):     Eye and face protective equipment being used did not meet the requirements specified in Z 87.1-1968:    South end of the building:  On or about November 7, 2012, two employees attaching metal panels to the walls of a system engineered building wore prescription eyewear that did meet the requirement of the American National Standard Insititute Standard (ANSI) Z 87.1-1968.  This exposed the employees to the hazard of being struck in the eyes by metal debris.

29 CFR 1926.451(a)(6):     Scaffolds were not designed by a qualified person:    South side of the building:  On or about November 7, 2012, and times prior to, employees erecting a metal system engineered building used a platform supported by a rough terrain forklift.  The platform had not been designed by qualified person, and its design and use was deficient.  This exposed the employees to the hazards of falling over 10 feet and/or failure of the forklift.

29 CFR 1926.451(c)(2)(iv):     Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    South side of the building:  On or about November 7, 2012, and times prior to, employees erecting a metal system engineered building used a platform supported by a rough terrain forklift.  The manufacturer instructions warned that the forklift was not designed to lift personnel.  This exposed the employees to the hazards of falling over 10 feet and/or failure of the forklift.

The forklift did not have load range charts.
