Humbolt St., Denver, CO 80204: On and before November 27, 2012, one employee working on 5:12 pitch, one story single family homes roof was not protected from falling by a system as described above.  Specifically, the employee was witnessed and photographed nailing plywood to the frame and trusses of the house.  Condition exposed employees to fall hazard of approximately 21 feet.     Abatement Note:  Abatement certification is required for this item (See enclosed Sample Abatement-Certification Worksheet).
