29 CFR 1910.178(a)(5):     If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    In the Production Area, a forklift operator atttaches a "carpet roll attachment" to the forklilft to lift a fiberglass mesh roll onto a platform and the forklift truck was not marked with the identity of the attachment and approximate weight of the truck and attachment at maximum elevation with load laterally centered.

29 CFR 1910.178(l)(1)(ii):     Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    An employee who operated a sit down forklift in the production area to go get sheets and bring them back to the production line had not completed the training as required by paragraph (l).

29 CFR 1910.215(a)(4):     Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    On the Production Line, a Cutter Operator was exposed to a struck-by hazard while using an abrasive wheel grinder with the work rest at 1/2 inch from the abrasive wheel while sharpening blades for the Cutter portion of the cement board production line.

29 CFR 1910.215(b)(9):     Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)    On the Production Line, the Cutter Operator was exposed to a struck-by hazard due to the lack of a tongue guard on the abrasive wheel grinder while sharpening blades for the Cutter on the cement board production line.

29 CFR 1910.305(g)(1)(iv)(A):     As a substitute for the fixed wiring of a structure;    On the Production Line, a 110 Volt AC flexible power cord attached to a stationary bench abrasiive wheel grinder was plugged into an extension cord set that was directly wired into the two receptacle electrical outlet.
