29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Slater Run Land Development, LLC doing business as Clear Fork Resort, located at 341 Resort Drive, Butler, OH 44822: On or about November 20, 2012, the employer failed to establish a lockout / tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, installing and making adjustments to equipment such as, but not limited to, SunKid Wonder Carpet conveyor (Type B750, Serial # SKD16112). Employees were exposed to amputation hazards while performing maintenance activities.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
