29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Excavation located off the shoulder of Newnan Bypass on Millard Farmer Industrial Boulevard, Newnan, GA 30265: Employees exposed to over head hazards inside an excavation which measured 12' long x 6' wide x 5' deep were not provided with head protection. Overhead hazards included mini-excavator bucket, 1 1/2" water hose, and hand tools.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Excavation located off the shoulder of Newnan Bypass on Millard Farmer Industrial Boulevard, Newnan, GA 30265: Employees engaged in 18" HDPE sewer line repair work in a 12' long x 6' wide x 5' deep excavation were not provided with a safe means of egress, i.e. ladder and/or ramp.
