29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a)Mason Development, Inc. @ 2374 W. 33rd Ave., Denver, CO 80204: On and before November 9th, 2012, the employer, as controlling employer at the job site, did not ensure that employees of the framing subcontractor wore protective helmets when exposed to overhead hazards such as, but not limited to, falling tools and materials.

29 CFR 1926.403(b)(1): Employer did not ensure that electric equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:      (a) New Future Construction, Inc. @ 2374 W. 33rd Ave., Denver, CO 80204:  On and before November 9th, 2012, the exposing employer did not ensure adequate insulation on an extension cord as it was missing approximately 1 inch of outer sheath insulation near the plug end.  The cordset was being used to power tools on the job-site in conjuction with framing operations. This condition exposed the employee to hazards such as electrical shock and burns.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a)New Future Construction, Inc. @ 2374 W. 33rd Ave., Denver, CO 80204:  On and before November 11th, 2012, the exposing employer did not ensure framing employees were protected from injury, in that employees conducting sheathing operations near the edge of the second story balcony and roof were utilizing a fall protection system. This condition exposed the employee to a falls of approximately 10 feet and 20 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    (a)New Future Construction, Inc. @ 2374 W. 33rd Ave., Denver, CO 80204: On and before November 9th, 2012, the exposing employer did not ensure that each employee engaged in framing operations on a new multi-family residence under construction, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet.
