29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: (a) Jobsite, VA Medical Center, Altoona, PA: On or about 11/28/12, the 110v dual outlet being used to supply power to the Abatement Technologies Portable Air Scrubber did not have a cover exposing employees to potential electrical shock hazards when plugging/unplugging the equipment when it is needed to be moved.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: (a) Jobsite, VA Medical Center, Altoona, PA: On or about 11/28/12, the wheels on the mobile scaffold were not locked. An employee was working from the scaffold taping off the work area and the wheels were not locked to prevent movement while the scaffold was used in a stationary manner.
