29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Yard rebar fab area: On 11/26/12 and at times prior thereto, the blade guard on a Baileigh Industrial horizontal blade guard was not adjusted to cover exposed blade not involved in the cutting operation. (b) Yard: On 11/26/12 and at times prior thereto, there was no guard to prevent contact with the ingoing nip point at the drive chain and sprocket for the drive wheel of a Mi-Jack Travelift 650C. (c) Mobile acid washer: On 11/26/12 and at times prior thereto, a belt and pulley ingoing nip point was not guarded. NOTE: Abatement certification IS required for items (a) and (b) and IS NOT required for item (c) which was corrected during the inspection.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Rebar Fabrication area - bender 1: On 11/26/12 and at times prior thereto, the outer cover of a flexible cable was broken away from the devise that prevents strain relief on terminal screws. (b) Rebar Fabrication area - bender 2: On 11/26/12 and at times prior thereto, the device that prevents strain relief on terminal screws was unscrewed from the rebar bending machines foot-pedal. (c) Rebar Fabrication area Wire feed welder: On 11/26/12 and at times prior thereto, the flexible cord male end fitting was loose and not providing strain relief on terminal screws. NOTE: Abatement certification IS required for this item.
