29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: On or about November 28, 2012, the employer, whose employees were performing foundation work, failed to provide fall protection. Two employees used a ladder to walk across an excavation to access the work area were exposed to the hazard of falling distances greater than 6 feet to the ground below.

The employees while standing/working on top of the foundation wall were exposed to the hazard of falling distances greater than 6 feet to the ground below.

The employees were exposed to the hazard of rebar impalements when accessing and egressing the work area.
