29 CFR 1910.212(a)(3)(ii):     Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Employees were exposed to amputation hazards while operating the AccurShear (Model #62506 and Serial #5234) and the employer did not ensure that the operator was properly guarded from the point of operation.    (b) Employees were exposed to amputations hazards while operating the Wells Metal Band Saw (Model #600 and Serial #10131), which was located in the weld shop, and the employer did not ensure that the operator was properly guarded from the unused portion of the blade.    (c) Employees were exposed to amputations hazards while operating the Jet Horizontal Band Saw (Model # and Serial # could not be determined), which was located in the weld shop, and the employer did not ensure that the operator was properly guarded from the unused portion of the blade.
