29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:    A. Continental Homes worksite located at 12052 US 20A, Wauseon, Ohio: On or about November 28, 2012, the employer did not ensure employees were protected from falls while removing weatherproofing on the two story residential structure. The employees were exposed to a fall distance of approximatley 20.00 feet from the 6/12 pitch roof.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    A. Continental Homes worksite located at 12052 US 20A, Wauseon, Ohio: On or about November 28, 2012, The employer did not ensure the side rails of the extension ladder was extended at least three feet above the landing surface. Two employees were exposed to falls while using the ladder to access the roof.
