29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.       On or about November 28, 2012, on the south side of a commercial building, employees installing H-metal material worked from a man-lift at a height approximately 30 feet from the ground. The employees were wearing personal body harnesses without a lanyard attached to the boom or basket to prevent a fall to the ground.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      On or about November 28, 2012, employees were exposed to a fall hazards of approximately 30 feet while installing H-metal roofing material on a commercial building with a 12:12 pitch roof without fall protection.
