29 CFR 1910.22(c):     Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.    a) Aeration lagoon - Employees perform a daily entrance into the lagoon area to stand or walk near the aeration lagoon to take water samples.  The lagoon 10-15 feet deep with a plastic liner and is not equipped with either a cover or guardrails, thereby exposing the employees to a drowning hazard, observed on or about 11-28-12.    b) Clarifier shed - An employee enters the building several times times a day and works near or on a traveling bridge above a tank containing 10-14 foot deep sewage.  The tank is not covered or guarded, thereby exposing the employee to a drowning hazard, on or about 11-28-12.       Abatement documentation required.

29 CFR 1910.23(c)(2):     Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent).      a) Dump Station  -  Employees unloading their vacuum trucks stood on runways where the railings were not complete. Railings were missing at  4'2" near the bar screen rake out and at 15 feet high near the barrel, thereby exposing the employees to a serious hazard, on or about 11-28-12.      Abatement certification required.

29 CFR 1910.23(d)(1)(ii):     Stairway(s) less than 44 inches wide having one side open, did not have at least one stair railing on the open side:    a) Dump Station - Employees used a set of 17 step high fixed metal stairs to access between the upper and lower ground levels.  The stairs had no stair railing on the open side thereby exposing the employees to a serious hazard,  observed on or about 11-28-13.    Abatement certificatioin required.

OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to a caught in hazard from the manually controlled fixed conveyors.    a) Employees operate a 2.5 meter Phoenix Belt Press to remove water from sewage sludge.  The conveyor was not equipped with an operable emergency stop cord, thereby exposing the employee to a serious hazard, observed on or about 11-21-13.     Among other methods, one feasible and acceptable means to correct these hazards is to comply with the requirements outlined in the NFPA 79 10.7.1.1 Electrical Standard for Industrial Machinery.     Abatement certification required.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts :    a) Employees operate a 2.5 meter Phoenix Belt Press to remove water from sewage sludge.  The conveyor had an area where the belt crossed a roller that was unguarded thereby exposing the employee to a hazard, on or about 11-21-13.     Abatement certificationn required.
