29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: a) Chase Bank, an employee in the process of grinding concrete was exposed to an approximate 20 foot fall hazard, in that, the employer did not provide a safety platform and or cage, observed on or about 11/29/2012.
