Tilting forward with load engaging means elevated shall be prohibited except to pick up a load. An elevated load shall not be tilted forward except when the load is in a deposit position over a rack or stack: a) Outside Area Adjacent to the Wastewater Scrubber: On or about 11/21/2012, an employee was operating a Lull, Model 944E-42, in an elevated position with a work platform attached and an employee on the platform. The load was tilted forward causing the attachment and the employee to fall to the ground approximately 20-30 feet.
