29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Lutheran Homes Society, Inc. dba Lutheran Memorial Home - Sandusky, Ohio: On or about December 3, 2012, the employer did not assure that equipment for flushing of the eyes was provided to employees who may be exposed to hazardous corrosive chemicals.  Employees that transfer cleaning chemicals to portable spray bottles were exposed to dimethyl benzyl ammonium chloride and dimethyl ethylbenzyl ammonium chloride found in the Quat-Clean and Virex cleaning solutions.  An eye wash station was not provided for emergency use within the work area where chemicals are transferred.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:    a)  Lutheran Homes Society, Inc. dba Lutheran Memorial Home - Sandusky, Ohio: On or about December 3, 2012, the employer did not conduct an annual review of the exposure control plan.  A review of technology for safer needles that would eliminate or reduce exposure to bloodborne pathogens and consideration and implementation of available and safer medical devices was not documented.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
