29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.     North side of the structure; employees installed windows from wooden scaffolds without the use of fall protection, exposing employees to fall hazard at approximately 20 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    North side of the structure; employees worked near the edge of the trusses to receive a window from a worker on a ladder below without the use of fall protection, exposing employees to fall hazard at approximately 10 feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.    North side of the structure; employees accessed a 10 feet portable extension ladder that did not extend 3 feet above the structures landing surface, exposing employees to fall hazards.
