Line 1 Dry Cure Ovens - On or about November 16, 2012, the employer did not ensure that the e-stop provided on the bake oven switchboard panel was operational prior to its use. Switchboards that have any exposed live parts shall be located in permanently dry locations and shall be accessible only to qualified persons.      a. Line #1 Dry Cure Ovens - On or about November 16, 2012,  unqualified employees were required to access the bake oven switchboard panel thereby exposing them to the hazards of live electrical parts.

29 CFR 1910.23(c)(3):     Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    a. Hook set up area - On or about November 16, 2012, an employee was required to work on an open sided platform adjacent to a moving conveyor hook line.  The conveyor hook line platform was not provided with a standard guard railing thereby exposing employees to hazards of moving parts.

29 CFR 1910.147(c)(6)(i):   The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a. Entire facility - On or about November 16, 2012, the employer had not conducted periodic inspections of established energy control procedures in place for employees who perform servicing and maintenance on such equipment including, but not limited to, the line dry cure ovens. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
