29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). a. Located at the receiving department, employees were required to operate forklifts in order to load and unload racking systems and shipping trucks without being trained on the safe operation of forklifts, thereby exposing the employees to the hazards associated with operating forklifts.

Located in the receiving department, there was an electrical service panel that was used in part to supply electrical power to the building that was blocked by store merchandise, thereby exposing the employees to electrical shock and/or fire hazards due to the service panel not being readily accessible in case of an emergency.

29 CFR 1910.1200(f)(5)(ii): Execept as provided in paragraphs (f)(6) and (f)(7) of this section, the employer shall ensure tht each container of hazardous chemicals in the workplace is labeled, tagged or marked with the following information: Appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. a. Located in the receiving department, there were weed killer containers stored that did not have any labels to indicate the container's contents, thereby exposing the employees to the hazards associated with the with the weed killer.
