29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about November 27, 2012 and times prior, the 50" Flexo Fold Gluer Machine, Counter Ejector: The employer failed to ensure employees entering the caged area around the counter ejector used proper lockout procedures to isolate the energy source from 220/480 electrical. The employees were trained to turn off the main power switch on the control panel and open the gate which has an interlock on it. Neither of these two steps isolates the main power from the counter ejector area, which expose employees clearing jams to the unexpected start up of equipment.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about November 27, 2012 and times prior, at the 50" Flexo Folder Gluer Machine: The employer failed to ensure employees were protected from in-going nip points of the counter ejector rollers when a lockout exceptions form was used to allow employees to enter the guarded area for the folding rails through an interlocked gate. Once the gate was opened the employee would close it and signal the operator to start the machine when adjustments to the guide rails were made. This exposed employees to the hazards of being caught-in the rollers should they attempt to clear a jam or make an adjustment on them.
