29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: (a) Construction Site: On or about November 30, 2012, and at times prior thereto, employees were exposed to a fall of approximately 12 feet to the ground below while installing steel angle iron exterior wall. Employees were standing on a sheet of OSB from the raised forks of a forklift.

29 CFR 1926.503(a)(1): The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: (a) Construction Site: On or about November 30, 2012, and at times prior thereto, employees were exposed to a fall of approximately 12 feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
