29 CFR 1910.304(b)(3)(ii)(C)(3):     Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired;    a)	On or about November 29, 2012; at the BP Yard area of the facility, the employer allowed employees to work near a welding machine and two temporary power distribution panels, which were connected by cord and plug and were not visually inspected before each days use for external defects.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a)	On or about November 29, 2012; on the east side of the BP Yard area, the employer allowed employees to use the Idealarc AC/DC 250 (S/N-U1051208351) welding machine to weld pipe together.  The welding machine electrical cord did not have adequate strain relief.        b)	On or about November 29, 2012; on the east side of the BP Yard area, the employer allowed employees to use a distribution transformer (S/N-J05M10023) which had an electrical cord that was not provided adequate strain relief.  The distribution transformer is part of a temporary power distribution panel that is used by employees to power portable grinders.      c)	On or about November 29, 2012; at the center of the BP Yard area, the employer allowed employees to use a distribution transformer, which had an electrical cord that was not provided adequate strain relief.  The distribution transformer is part of a temporary power distribution panel that is used by employees to power portable grinders.

29 CFR 1910.305(b)(1)(i):     Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.    a)	On or about November 29, 2012; on the east side of the BP Yard area, the employer allowed employees to work near a Square D safety switch that had a conductor entering the cutout box which was not protected from abrasion, and an the opening through which the conductor entered was not effectively closed.
