29 CFR 1926.252(a):Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used: (a) 68 North Pleasant St Amherst, MA: On or about November 20, 2012, employees were observed throwing/pushing roofing materials they had stripped from the roof of this building into a dumpster 25 feet below the roof eave.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 68 North Pleasant St Amherst, MA: On or about November 20, 2012, employees were stripping shingles from this steep roof (slope 6/12 and 12/12) more than 25 feet above the ground below without any fall protection as required by this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
