29 CFR 1926.304(d):  When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position:    a)   At 685 Banbury Street Auburn, Alabama:  On or about November 28, 2012 and at times prior; the employer exposed his employees to amputation hazards in that he allowed them to use a Makita circular saw when the guard had a piece of wood wedged in it to keep the guard in the up position exposing the blade.

29 CFR 1926.451(b)(2):  Scaffold platforms and walkways were not at least 18 inches wide:    a) At 685 Banbury Street Auburn, Alabama:  On or about November 28, 2012 and at times prior; the employer exposed his employees to fall hazards in that he did not ensure a homemade wooden scaffold had an adequate walking / working surface exposing his employees to falls of 19.5 feet to the ground below.
