29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.      a) On or about November 28, 2012, in the mill department, employees who wore prescription lenses that were not ANSI approved safety glasses while working on a Harig 618 Ballway Surface grinder, exposing employees to airborne objects.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.      a) On or about November 28, 2012, in the tooling department, employer did not develop the written lockout/tagout program when employees are servicing equipment, equipment, such as but not limited to Harig 618 Surface Grinder and the ErgoTech Injection Molding machines, exposing employees to the hazard of electrical shock.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.      a)On or about November 28, 2012, in the tooling department, no work rests were provided on the Milwaukee Bench Grinder, exposing employees to the hazard of being struck by the wheel.
