29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a)  At 296 Bentley Court Lot 102 Auburn, Alabama:  On or about November 29, 2012 and at times prior; the employer failed to protect his employees from fall hazards in that he did not ensure floor openings were guarded to prevent employees from falling 11.2 feet to the floor below.    b)  At 296 Bentley Court Lot 102 Auburn, Alabama:  On or about November 29, 2012 and at times prior; the employer failed to protect his employees from fall hazards in that he did not ensure floor openings were guarded to prevent employees from falling 11.4 feet to the floor below.

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  At 296 Bentley Court Lot 102 Auburn, Alabama:  On or about November 29, 2012 and at times prior; the employer failed to ensure employees involved in framing activities were using fall protection exposing them to falls of 9 feet to the floor below.    b)  At 296 Bentley Court Lot 102 Auburn, Alabama:  On or about November 29, 2012 and at times prior; the employer failed to ensure employees involved in framing activities were using fall protection exposing them to falls of 22.8 feet to the ground below.
