29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about November 14, 2012; second floor, employees were using portable extension cords that had the ground pin missing to power portable equipment such as but not limited to circular saws.    No abatement certification or documentation required.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about November 14, 2012; second floor eastside; employees were observed working on the top plate of a two story new residential home without a personal fall arrest system in place to prevent them from falling 25-30 feet to ground level.    b) On or about November 14, 2012; second floor eastside stairway; employees were working around open working/walking surfaces within the new residential construction that were not properly guarded by a guard rail system; exposing them to a 10-12 foot fall to the lower level.    c) On or about November 14, 2012; throughout second floor area; employees were working around wall openings that were not properly guarded to protect them from falling 12-15 feet to ground level.    No abatement certification or documentation required.
