(a) On or about November 28, 2012 at 9400 NE 91St Liberty Missouri 64068 on the Northeast side of the house, employees were exposed to a hazard by working on a scaffold 13 feet in height placed on unstable objects, to include bricks and wood squares that were absent of base plates.
