Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    (a)  Employees installing roof insulation on a 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12.6 feet.

29 CFR 1926.453(b)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    (a)  An employee working from a JLG 660SJ aerial lift was not protected from a fall/thrown hazard by using a safety harness and lanyard attached to the lift.
