29 CFR 1926.1101(f)(1)(i):  Where exposure monitoring was required under this section, the employer did not perform monitoring to determine accurately the airborne concentrations of asbestos to which employees were or potentially were exposed:    On or about December 11, 2012, at the site located at 25089 Center Ridge Road, Westlake, Ohio, employees removed laboratory benches which contained asbestos panels.  This work was defined as Class 2 asbestos removal.  Employees were not monitored to determine their exposure to asbestos.

29 CFR 1926.1101(k)(3)(ii)(B): The employer did not inform employees who performed work under 29 CFR 1926.1101 of the location and quantity of asbestos containing material and/or presumed asbestos containing material present in the area:    On or about January 10, 2013 at the site located at 25089 Center Ridge Road, Westlake, Ohio, employees removed laboratory benches which contained asbestos panels. The employer did not inform  his demolition employees of this required information.

29 CFR 1926.1101(k)(9)(i): The employer did not institute, at no cost to employees, a training program for all employees who were likely to be exposed in excess of the PEL and for all employees who performed Class I through IV asbestos operations:    On or about December 11, 2012, at the site located at 25089 Center Ridge Road, Westlake, Ohio, employees removed laboratory benches which contained asbestos panels.  This work was classified as Class II asbestos work.  No asbestos training was given to these employees.
