29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) Ricor Inc, at Ogden Street and 58th Avenue, Denver, CO: On 11/29/12 Ricor Inc did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  Approximately five employees were installing a water line in excavations adjacent to 58th Avenue.  The excavations extended to the roadway.  The work zone established extended into the roadway.  The employer did not provide work zone training to all employees.  This condition exposed employees to a struck by hazard.    (b) Ricor Inc, at Ogden Street and 58th Avenue, Denver, CO: On 11/29/12 Ricor Inc did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  Approximately five employees were installing a water line in excavations adjacent to 58th Avenue.  The excavations ranged in depth from 4.9 feet to 6 feet.  The employer did not provide excavation training to all employees.  This condition exposed employees to an excavation hazard.

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA, which are incorporated by reference.    (a) Ricor Inc, at Ogden Street and 58th Avenue, Denver, CO: On 11/29/12 Ricor Inc did not ensure that traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA, which are incorporated by reference.  Approximately five employees were installing a water line in excavations adjacent to 58th Avenue.  The excavations extended to the roadway.  The work zone established extended into the roadway.  Traffic control devices, ie cones, separated the employee work area from the 35 mph speed limit lane used by semi truck traffic.  The traffic lane as marked by the cones ranged in width from 8' 9"  to 10' 2".  The width of the lane was inadequate for the size and speed of traffic on 58th Avenue.  The MUTCD and the employer's traffic control plan required a minimum lane width of 10 feet.  This condition exposed employees to a struck by hazard.

29 CFR 1926.651(e): Employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials:    (a) Ricor Inc, at Ogden Street and 58th Avenue, Denver, CO: On 11/29/12 employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials.  Approximately five employees were installing a water line in excavations adjacent to 58th Avenue.  One employee was standing within a 4.9 foot deep excavation while a backhoe was removing soil.  The employee was not required to stand away from the backhoe while it was loading or unloading.  This condition exposed the employee to a struck by hazard.

Approximately five employees were installing a water line in excavations adjacent to 58th Avenue.  Two employees worked within an approximate 6 foot deep excavation with vertical sides.  The competent person classified the soil as Type C soil.  Neither shoring, trench box, nor other protective system were in use.  The employer did not ensure that the employees were protected from a cave-in by a protective system.  This condition exposed the employees to an excavation hazard.
