29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about November 30, 2012, at the Joint Task Force North Compound, Building 11677, three employees were observed working along the edges of a flat roof without any form of fall protection. Employees were exposed to a fall hazard of 14-feet 3-inches to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about November 30, 2012, at the Joint Task Force North Compound, Building 11677, three employees were utilizing an extension ladder to access the roof. The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. The employees were exposed to a fall hazards.
