29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) Tactical Cleaning Company, at 3400 E 56th Avenue, Commerce City, CO: On 11/16/12, Tactical Cleaning Company did not ensure that the path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. Employees were preparing to enter a railcar containing asphalt. A halogen worklight was used to provide light. The worklight was connected via an extension cord to an outdoor receptacle. The circuit was live. The ground pin for the flexible cord of the worklight was missing. The employer did not ensure that the path to ground for the worklight was continuous. This condition exposed employees to an electrical hazard. Tactical Cleaning Company, 3400 E 56th Avenue, Commerce City, CO, was previously cited for a violation of this Occupational Safety and Health Administration Standard, 29 CFR 1910.304(g)(5) which was contained in Item 11 of Citation 1 issued on September 30, 2010 (OSHA Inspection number 314657834).

29 CFR 1910.146(f)(7): The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered: (a) Tactical Cleaning Company, at 3400 E 56th Avenue, Commerce City, CO: On 11/16/12, Tactical Cleaning Company did not ensure that the entry permit that documented compliance and authorized entry to a permit space identified the hazards of the permit space to be entered. On 11/16/12 one employee entered a railcar that had contained gasoline. This space was a permit required confined space (PRCS). The PRCS entry permit did not identify the hazards of the permit space to be entered. This condition exposed employees to a permit required confined space hazard. Tactical Cleaning Company, 3400 E 56th Avenue, Commerce City, CO, was previously cited for a violation of this Occupational Safety and Health Administration Standard, 29 CFR 1910.146(f)(7), which was contained in Item 7B of Citation 1 issued on September 30, 2010 (OSHA Inspection number 314657834).
