29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. During work on saws, chippper, and electrical, although equipment was powered down, a lock was not placed on the disconnecting means. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a. After a change of equipment the employer had not determined employee exposure to determine the need for a hearing conservation program. During the inspection, sound level meter readings indicated exposure greater than 85 dBA for eight hours. (Note: abatement may be made by re-instituting annual hearing tests and training without first conducting eight hour time weighted average noise measurements). Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
