29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal): a) Route 440 North at New Hook Road, Bayonne, NJ Employees laying pipeline in the 206 foot long, 10 to 11 foot deep trench, were exposed to struck-by and crushing injuries from a cave-in as the trench was not adequately sloped or protected by shields or shoring in the areas where employees were working. Violation observed on or about 10/24/2012.. HENKELS & MCCOY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT 29 CFR 1926.652(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315060855, CITATION NUMBER 2, ITEM NUMBER 1, ISSUED ON 08/11/2011, FINAL ORDER DATE 04/15/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 464 RIDGE AVE., LAKEWOOD, NJ.
