29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about November 29, 2012, and at times prior thereto, at the jobsite located at 833 Beechwood in Fayetteville, Arkansas, the employer allowed employees to work from a forklift mounted platform that had not been approved by the fork lift manufacturer, exposing employees to the hazard of falling approximately 24 feet to the ground below.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about November 29, 2012, and at times prior thereto, at the jobsite located at 833 Beechwood in Fayetteville, Arkansas, the employer allowed employees to move the rough terrain forklift and platform, horizontally with personnel on the platform, exposing employees to the hazard of falling approximately 24 feet to the ground below.
