29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Jobsite: Telephone Way, Pittsburgh, Pennsylvania, 15232: On or before November 29, 2012, employees were working unprotected and exposed to fall hazards of approximately 18 feet to the ground below. The slope of the roof ranged from flat to 4 in 12 (vertical to horizontal). The employees were applying roofing materials.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) Apartment Building Roof, Telephone Way, Pittsburgh, Pennsylvania, 15232: On or about November 29, 2012, the employer did not retrain employees who were observed performing roofing work on a three story apartment building without the use of fall protection. The employees were exposed to a fall hazard of approximately 18 feet to the ground.
