Beechwood St. in Fayetteville, Arkansas, the employer did not ensure the tubular welded frame scaffold was fully planked, exposing employees to the hazard of falling from heights of 6 to 26 feet to the ground below in the following locations:    a) The fourth level at the northwest corner of the building.  b) The second level at the northwest corner of the building.  c) The third level three frames east of the northwest corner of the building.  d) The third level four frames east of the northwest corner of the building.  e) The first level at the front of the northwest corner of the building.  f) The fourth level five frames east of the northwest corner of the building.  g) The third level of the fames on the front wall at the northeast corner of the building.  h) The fourth level of the fames on the front wall at the northeast corner of the building.  i) The fourth level seven frames from the south end of the scaffold along the east wall.  j) The fifth level at the southernmost end of the scaffold along the east wall of the building.  k) The fourth level six frames from the southernmost end of the scaffold along the east wall of the building.  l) The fourth level one frame from the southernmost end of the scaffold along the east wall of the building.  m) The fourth level at the southernmost end of the scaffold along the east wall of the building.  n) The fourth level five frames from the southernmost end of the scaffold along the east wall of the building.  o) The fourth level two frames from the southernmost end of the scaffold along the east wall of the building.

On or about November 29, 2012, at the jobsite located at 833 S. Beechwood in Fayetteville, Arkansas, the employer did not ensure each platform unit was installed so that space between adjacent units and the space between the platforms and the uprights were not more than one inch wide, exposing employees to struck by hazards and the hazard of falling from heights of 6 to 26 feet to the ground below in the following:    a) The fifth working level at the northwest corner of the building.  b) The walk way at the second level connecting the fourth and fifth frames from the northwest corner of the building.  c) The third working level at the sixth frame from the northwest corner.  d) The fifth working level, on the east wall, one frame from the northeast corner of the building.  e) The walkway at the fifth level, on the east wall, connecting the first and second frames from the northeast corner of the building.

Beechwood St. in Fayetteville, Arkansas, the employer allowed ends of platforms more than10 feet in length to extend more than 18 inches over its support, exposing employees to the hazard of falling from heights of 6 to 21 feet to the ground below in the following:    a) The fourth level of the scaffold at the northwest corner of the building.  b) The second level of the scaffold at the northwest corner of the scaffold.  c) The first level of the scaffold at the northwest corner of the building.  d) The first level of the third frame from the northwest corner of the building.  e) The second level of the third frame from the northwest end of the building.  f) The fourth level of the second frame from the northwest corner of the building.  g) The fourth level of the fifth frame from the northwest corner of the building.  h) The second level of the scaffold at the northeast corner of the building.  i) The third level of the scaffold at the northeast corner of the scaffold.  j) The fourth level of the scaffold at the northeast corner of the scaffold.

Beechwood St in Fayetteville, Arkansas, the employer had not developed, implemented and enforced programs that included frequent inspections of the scaffold system, exposing employees to the hazard of scaffold collapse and the hazard of falling from heights of 6 to 26 feet to the ground below.
