29 CFR 1910.101(b):     The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    a) Machine Shop, Pulzar Laser Machine, Nitrogen Compressed Gas Cylinder -- On or about November 28, 2012, an employee was tasked to work around an unsecured compressed gas cylinder, exposing employees to a struck-by hazard.     ABATEMENT NOTE: Compressed Gas Association Pamphlet P-1-1965 paragraph 3.4.4 states the following: Before using cylinder, be sure it is properly supported to prevent it from being knocked over.

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) Machine Shop, Chicago Dries & Krump Mechanical Brake Machine -- On or about November 28, 2012, employees operate industrial machinery that is routinely serviced, but written lockout/tag out program had not been developed that would dictate contractor requirements and provide guidance for affected level employee training.

29 CFR 1910.147(f)(2)(i):     When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    a) Machine Shop -- On or about November 28, 2012, employees operate industrial machinery that is routinely serviced by outside contractors, but a written lockout/tag program had not yet been developed to direct affected level employees on the proper procedure needed in the event of machine abnormality or failure.
