29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:     On or about November 28, 2012, and at times prior thereto, the employer did not verify through a written certification that an assessment of the workplace had been performed to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE) for employees  such as, but not limited to, Assemblers and Metal Smiths, engaged jewelry production, finishing and assembly operations which include work operations such as, but not limited to, soldering, metal abrasion, kiln firing, and pickling.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about November 28, 2012, suitable facilities for quick drenching or flushing of the eyes and body, such as an emergency shower/eye wash, were not provided in the work area for Metal Smiths using Sparex No. 2 [Sodium bisulfate (>90%); Sodium sulfate (4.8-8%)] in solution to pickle metal jewelry pieces.
