29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: a/ Adjacent to Furnace #5, a "long rake" approximately 30 feet long was secured to the forks belonging to a Toyota Fork Truck, m/n 7FDNU50, s/n 70557. Neither the manufacturer's written approval was given in advance nor were the plates, tags, or decals on the fork truck changed accordingly.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a/ Adjacent to Furnace #5, a "long rake" approximately 30 feet long was secured to the forks belonging to a Toyota Fork Truck, m/n 7FDNU50, s/n 70557. The fork truck was not marked accordingly.
