29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: a) 167 Pine Hill Road, Feasterville, PA, Worksite - An employee sustained a work-related injury on November 21, 2012 which resulted in the employee's death on December 2, 2012 and the employer did not report the fatality to OSHA within eight hours of the death of this employee, on or about December 4, 2012.
