29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) R-2 Rolling Mill Area - On or about November 29, 2012, employees were exposed to the floor being coated in slippery accumulations of lubrication oil for a radius of more than 10 feet beyond the operators station in front of the Rolling Mill. b) R-3 Rolling Mill Area - On or about November 29, 2012, employees were exposed to the floor being coated in slippery accumulations of lubrication oil for a radius of more than 10 feet beyond the operators station in front of the Rolling Mill. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): a) R-2 Rolling Mill Area - On or about November 29, 2012, employees were exposed to the floor being coated in slippery accumulations of lubrication oil for a radius of more than 10 feet beyond the operators station in front of the Rolling Mill, and no safety instruction signs were posted to warn employees traveling in the area of the slippery condition of the floors. b) R-3 Rolling Mill Area - On or about November 29, 2012, employees were exposed to the floor being coated in slippery accumulations of lubrication oil for a radius of more than 10 feet beyond the operators station in front of the Rolling Mill, and no safety instruction signs were posted to warn employees traveling in the area of the slippery condition of the floors.
