29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about November 29th, 2012 in the machine shop area of the plant employees were exposed to serious injuries from a number of rotating parts, in-running nip points, pinch-points, belts and pulleys, and other machine hazards in that a number of machines were not adequately guarded against accidental contact by the employees.  These machines included, but were not necessarily limited to, all of the taper machines and vertical drill presses (which lacked chuck guards).  Such contacts and potential entanglements could result in strains, sprains, broken bones, lacerations, crushing-type injuries and possible amputations.      Abatement certification for this violation is required.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      On or about November 29th, 2012 in the maintenence shop area of the plant employees were exposed to serious injuries from a number of rotating parts, in-running nip points, pinch-points on an 8 inch bench grinder that had a tongue guard whose spacing was greater than 1.5 inches.     Abatement certification for this violation is not required.

29 CFR 1910.305(b)(2)(i):     Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:      On or about November 29th, 2012 in the machine shop area of the plant employees were exposed to a serious electrical hazard due to an electrical outlet not have any cover or faceplate and therfor exposing the employees to electrical shock and burn or even electrocution hazards.     Abatement certification for this violation is not required.
